Information necessary to procure a full PEO proposal

As HROplus takes its clients through the process of finding the right solution, it is necessary for certain data to be obtained. Below is a list of the basic information that is necessary in order for HROplus to make a full evaluation of current human resources costs and functions of your company, and make a full assessment of needs and placement within our network of providers that is in the best long-term interest of our clients.

Info needed from a company that is currently operational:

1. Recent copy of payroll with workers comp code for each employee
2. Most recent quarterly SUTA Statement
3. Copy of current healthcare plan description and rates and an employee census
4. Copy of your Current Workers Compensation Declaration (or Dec) Page
5. Workers Compensation loss runs for the past three years
6. Tax ID and corporate status (i.e. C-Corp, S-Corp, LLC, etc.)

Info needed from a company that is a startup:

1. Business name and description, include corporate and dba names
2. Estimated payroll broken down by job description and location(s)
3. Tax ID
4. Employee census (if at least three employees are set to be hired)
5. Synopsis of Business Plan

Info needed from a company that is currently with a PEO or leasing company:

1. Invoice from current PEO or leasing provider
2. Workers compensation loss runs for the past 3 years or signed Loss Affidavit
3. Copy of your current healthcare plan description and rates
4. Tax ID and corporate status (i.e. C-Corp, S-Corp, LLC, etc.)

In addition, HROplus is comfortable with certain non-disclosure agreements and an agreement may be requested from HROplus at any time during the engagement.
